What is Functional Frenuloplasty?

Frenectomy or Frenuloplasty, is a relatively minor surgical procedure performed to loosen or remove overly large, tight, or poorly positioned band(s) of tissue that are present inside the mouth, connected to the lip, cheeks or floor of the mouth (frenum) – a condition commonly referred to as being “tongue-tied” or “ankyloglossia” – which can cause speech impediments and difficulties with chewing, swallowing and other aspects of oral function.
In infants, this condition can impair feeding, and for some older children, teens and adults, cause daily discomfort/pain and lead to mouth breathing. Once the tissue has been removed, the wound will typically be sutured. Frenuloplasty is a quick procedure, generally taking less than 45 minutes, and is often performed with just local anesthesia in adults.

Frequently Asked Questions

What is a functional frenuloplasty?

A functional frenuloplasty combines a precise tongue‑tie release with pre‑ and post‑procedure therapy (e.g., myofunctional exercises) to improve tongue mobility, posture, nasal breathing, and overall oral function. It’s designed for long‑term functional improvement—not just a quick release.

Who is a good candidate?

Older children, teens, and adults with restricted tongue movement and symptoms like mouth breathing, snoring, speech fatigue, TMJ/jaw tension, neck/shoulder tightness, orthodontic relapse, or difficulty maintaining tongue‑to‑palate posture may benefit. We perform a comprehensive exam and functional assessment to confirm candidacy.

What does the procedure involve?

After pre‑therapy and planning, the release is performed in‑office (often with a CO2 laser) under local anesthesia. The approach prioritizes precision, minimal bleeding, and optimal wound shape; sutures may be placed when indicated. You’ll receive detailed aftercare and a plan for post‑release exercises to support healing and prevent reattachment.

What is recovery like and how do I prevent reattachment?

Expect mild soreness or swelling for 24–72 hours. Gentle stretches/exercises, good oral hygiene, and follow‑up visits are key to proper healing and preventing reattachment. Most patients resume normal routines quickly, and we coordinate with your therapy team as needed.
